Iron dextran is used to treat iron deficiency anemia when oral iron supplements are not effective or cannot be tolerated. The dosage is calculated based on the patient's weight and the desired increase in hemoglobin levels.

• Adults and Children over 15 kg (33 lbs):
  • Dosage Calculation:
    Dose (mL)=0.0442×(Desired Hb−Observed Hb)×LBW+(0.26×LBW)
    • Desired Hb: Target hemoglobin level in g/dL (typically 14.8 g/dL for adults)
    • Observed Hb: Current hemoglobin level in g/dL
    • LBW: Lean body weight in kg (use actual body weight if lower than LBW)
      • For males: LBW = 50 kg + 2.3 kg per inch over 5 feet
      • For females: LBW = 45.5 kg + 2.3 kg per inch over 5 feet
• Children 5 to 15 kg (11 to 33 lbs):
  • Dosage Calculation:
    Dose (mL)=0.0442×(Desired Hb−Observed Hb)×Weight+(0.26×Weight)
    • Desired Hb: Target hemoglobin level in g/dL (typically 12 g/dL for children under 15 kg)
• Maximum Daily Dose:
  • Infants under 5 kg: 0.5 mL per day
  • Children under 10 kg: 1 mL per day
  • All others: 2 mL per day

Administration

Iron dextran can be administered either intravenously (IV) or intramuscularly (IM).

• IV Administration:
  • Administer doses of 2 mL or less per day.
  • A test dose of 0.5 mL should be given first, and the patient should be observed for at least 1 hour before administering the therapeutic dose.
  • Inject slowly to avoid adverse reactions.
• IM Administration:
  • Use the Z-track technique to inject deeply into the upper outer quadrant of the buttock.
  • Do not inject into the arm or other exposed areas.

Storage

• Store iron dextran at room temperature (20°C to 25°C; 68°F to 77°F) with excursions permitted to 15°C to 30°C (59°F to 86°F).
• Keep the solution away from moisture and heat.
• Do not use if the solution has changed colors or has particles in it.

Precautions

• Allergic Reactions: Iron dextran can cause serious allergic reactions, including anaphylaxis. Always have emergency medications available.
• Monitoring: Regularly monitor hemoglobin, hematocrit, and iron storage parameters.
• Discontinue Oral Iron: Stop taking oral iron supplements before starting iron dextran therapy
